Wiltshire Council is passionate about working together to find innovative ways to achieve the best possible outcomes for all our residents.
The Families and Children’s Transformation (FACT) partnership continues to build on previous work focussed on Contextual Safeguarding and integrating support for those with multiple and complex needs, identifying early intervention and prevention as its key focus for multi-agency transformation activity in the post-pandemic landscape. Linking closely with existing transformation activity associated with Transitional Safeguarding, Inclusive Approaches to Education and the emerging Integrated Care System (ICS) developments, the partners have committed to a programme of system-wide activity aimed at enabling social mobility and managing the demand for higher cost later intervention.
Similarly, across our adult services we are focussed, post pandemic and alongside our ICS colleagues, on strengthening and developing preventative services and approaches that support people to live full lives in their communities, drawing on and contributing to local, community-based support where appropriate, with the aim of reducing or delaying a need for formal care and support.
